Effective 1/21/2025, Good Shepherd Children’s Center has partnered with Five Star System. This system offers online forms and data management and replaces paper forms. You may review your data at any time to verify that it is current. This applies only to the Child History, Financial Statement, Emergency Forms, Child Release, and Information to Parents. You must complete the registration form (above) first in order to be entered into the 5 Star System.
You will receive an invitational email from Five Star Registration Systems to set up your account. Please add “Five Star Registration System” to your safe list.
